How to Submit a Referral for Guardianship or Payee Services

A referral is simply a conversation starter. Anyone can make one: a family member, a social worker, a hospital discharge planner, an attorney, a care provider, or a person asking for help with their own affairs.

What helps us respond quickly

  • The individual's name and county of residence, if known
  • Your relationship to them
  • The decision or task that is not getting handled
  • Any court date, hospital discharge, or benefit deadline involved
  • The best way and time to reach you

You do not need a diagnosis, a court order, or a complete history to start. If you have paperwork, we will ask for it at the right stage.

What happens after you submit

We review the information and contact you to understand the situation. From there we explain the options, including alternatives to guardianship, and what the process would require. If we are not the right fit, we will tell you and point you toward a better resource.

Confidentiality

Referral information is used only to evaluate and provide services. We do not share details with other agencies without a legal basis or written permission.

Timing

Guardianship matters often move on a court calendar, and payee matters depend on SSA processing. Earlier referrals give everyone more room to plan, especially before a hospital discharge or a lease expiration.
